PROCEDURES FOR FILING A COMPLAINT

1. The complainant should go to the Waukegan Police Department.
2. The complainant should speak to a Commanding Officer in regards to the
complaint.
3. The Commanding Officer will complete a "Citizen Complaint form.
4. The complainant should review the Citizen Complaint form to ensure that the
information is accurate.
5. The complainant must sign the Citizen Complaint form.
a. A copy of the Citizen Complaint form will be given to the complainant.
b. This copy will be the person's receipt of the complaint and will be used
for tracking purposes.
6. An affidavit will also be given to the complainant in compliance with Section 3.8
(b) of the Illinois Uniform Peace Officer's Disciplinary Act, 50 ILCS 725/1 et
seq.
a. The complainant must have the affidavit signed, notarized, and returned to
the Waukegan Police Department.
b. An investigation cannot be started until the affidavit is signed, notarized,
and returned to the Waukegan Police Department.
7. The affidavit is returned to the Waukegan Police Department.
8. The investigation into the complaint is initiated.
9. The Commander of the Office of Professional Standards or his designee will
contact the complainant:
a. with in fourteen days in order to update the complainant on the status of
the complaint.
b. at the end of the investigation in regards to its final results.
